This document is an Air Force UFO sighting report form (AFFORM 112) used to collect witness testimony about unidentified aerial objects. The form comprises structured questionnaires across multiple sections requesting detailed information about an observed sighting. Witnesses provide data on observation date, time, and location; environmental conditions (weather, visibility); object characteristics (appearance, color, luminosity); movement patterns and speed; duration of sighting; and behavioral changes. The form includes questions about observer location and activity context, visual conditions (whether sighting was through windows or unaided), and comparative object sizing. Later sections request witness sketches of the object's shape, trajectory path, and position markers. Witness personal information—name, address, age, occupation, education level—is collected along with corroborating witness names. The handwritten entries visible include observer comments describing color changes and movement observations, with remarks about accompanying witnesses.
