Pulp Magazine Cover

This document is not a pulp magazine but rather a U.S. Air Force Technical Information Sheet dated April 21, 1955, designed to collect witness reports of unidentified aerial phenomena. The form originates from the Air Technical Intelligence Center at Wright-Patterson Air Force Base, Ohio, and addresses correspondence from Hendersonville, North Carolina. The questionnaire systematically gathers detailed witness accounts through forty-two numbered questions covering observation circumstances, object characteristics, movement patterns, and witness credentials. Questions address timing, sky conditions, object brightness, motion (including whether it remained stationary, accelerated, changed shape, or pulsated), size estimation using comparative objects, material composition, direction of observation, speed, and distance estimates. The form requests sketches of object shape and motion trajectory. Witness information sections collect name, address, telephone, occupation, age, sex, and educational background. Much of the OCR is illegible, particularly in handwritten response sections, limiting detailed content recovery.
